Create your own personalized UHN/NPP directory on this website. Simply go to find a provider and complete a search. Once you locate your provider, click the "add to my personalized directory" button. The data will be placed in a file you can print before exiting the page. After you have selected your preferred hospital and physicians, you can print the directory. When you sign off, your personal directory will be saved. Each time you access your personal directory you will be notified that there has been changes to your existing provider selection allowing you to keep your personal directory updated.
Hard copy directories may be available at your employer's human resources department.
